Use mailout.php.net instead of php-smtp2.php.net

This is an alias for the current php-smtp3.php.net name.

Refactor long array() syntax to short []

Since app is using PHP 5.4+ already, the longer `array()` syntax can be
refactored to shorter `[]`.
