local header files, which are located next to source files in the source

Reorganize local header files

Apart from public and internal header files, there is a third type called
local header files, which are located next to source files in the source
directory. Currently, they have different suffixes like

'*_lcl.h', '*_local.h', or '*_int.h'

This commit changes the different suffixes to '*_local.h' uniformly.

RT4660: BIO_METHODs should be const.

BIO_new, etc., don't need a non-const BIO_METHOD. This allows all the
built-in method tables to live in .rodata.

Get rid of more non-ANSI declarations.

Move the registration of callback functions to special functions
designed for that. This removes the potential error to mix data and
function pointers.

and that's it, but it may be more correct to generate a genuine error
