Migrate skip checks to --EXTENSIONS--, p3

Extensions migrated in part 3:
* ftp
* gmp
* iconv
* opcache
* shmop

Fixed bug #79535

We did not allocate a cache slot for FETCH_CLASS. This is already
fixed on newer PHP versions.
