Migrate skip checks to --EXTENSIONS--, p3

* ftp
* gmp
* iconv
* opcache
* shmop

Improve undefined variable error messages

Closes GH-5312

Fixed bug #79412 (Opcache chokes and uses 100% CPU on specific script).
