Fix release build

- Prepare for optimization - use a single fetch for multiple resources
- Clean a bit

Portable use of multiple arguments macro without generating lots of
warnings when doing nothing.

Support for SGI's State Threads. Will be used by PHP's thttpd SAPI
initially.
