Made ob_start() and friends reentrant. It's now possible to implement this
long-requested functionality, now that output buffering is re-entrant:

Rename macros which begin with underscore to appropiate macros. The general
rule is:

macro_name=`echo $filename|tr a-z A-Z|sed 's/\./_/'`
