Rewrote exception support. Fixes a few limitations and bugs in the old
implementation, and allows exceptions to 'fire' much earlier than before.

Fix __autoload() with derived classes

Fixed bug #26697 (calling class_exists on a nonexistent class in __autoload
results in segfault).
