Remove deprecated call-time reference

* all tests should pass now (expect for the interbase stuff that I haven't
had the chance to test)

* Added "test" make target so you can do "make test" in any directory.
* Added copyright header to run-test.php

Took the old PHP 3 regression testing framework and rewrote it in PHP.
Should work on both Windows and UNIX, however I have not tested it on
